  • Adult First Time Applicants (Age 16 Years and Older)

Adult Passport Card for those who do not currently hold a valid Passport Book or Card

Note: First time applicant is required to apply personally at an authorized Passport Agency by the Acceptance Facility for Passport Application of the Embassy or Consulate of the United States.

Among other documents, the issuing authority will ask applicant to submit a passport 2”X2” photo graph. Have your photograph taken at CVS passport photo to guarantee that your passport photo follows the guidelines that are pretty much similar worldwide as they know all government requirements for passport.

Every applicants applying for Form DS 11 must pay a passport execution fee to recover the costs of completing the passport application for (1) oath administration: (2) applicant’s identity verification; and (3) transmission of application. The execution fee must be paid at the time of accomplishing the application.

All Minor applicants, also for renewals, must personally apply at an authorized Passport Agency or Acceptance Facility of the Embassy or Consulate of the United States.

Every applicant applying for Form DS 11 pays the same amount of passport execution fee that is used to recover the costs of executing the passport application.

Expedited Service fee is $60.00 for each application, aside from the required fees noted above. The Expedited Fee is required for each application and submitted in person at a Passport Agency or Center.

Overnight delivery of Passport Book for only U.S. applicants is $14.85 plus the required fees stated above.

File Search fee of $150.00 is required when applicant is unable to present evidence of citizenship.

Note: All application fees are non-refundable and the amount is retained by the Department of State whether or not the passport is issued.

Methods of Payment Accepted

  1. Credit Cards (American Express, Discover, Master Card and VISA) and Debit/Check Cards
  2. Checks (cashier, credit, personal, travelers), payable to “Department of State”
  3. Money orders (exchanges in currency, international, U.S. Postal) also bank drafts, to be paid to the “U.S. Department of State”
  4. Cash (exact change only)
